Annual Health and Medical Record Needed for Summer Camp

The Annual Health and Medical Record Needed to be completed for Summer Camp annually!

All sections, including A, B and especially C (updated by doctor) need to be completed.  So, please schedule a physical with your scout's doctor/pediatrician if you have not already done so.

All scouts and leaders need to have a completed Annual Health and Medical Record.  Any leaders spending the night should complete sections A and B, but any leaders staying less than 72 hours do not need to complete section C.
